INTERIM REPORT SIGNIFICANT CONSTRUCTION DEFICIENCY No. 54

- TUBE STEEL.(HT 16600) LAMINATIONS"

_ INTRODUCTION This report is submitted pursuant to 10CFR50.55(e) . It describes a condition discovered on material (Tube Steel HT-16600) and specifically identifies ob-vious laminations observed on uninstalled pieces. This problem is considered:

reportable under the requirements of 10CFR50.55(e). To.the best of our know-ledge, this problem has not been identified to the Nuclear Regulatory Commis-sion pursuant to 10CFR21.

DESCRIPTION Mercury Company of. Norwood, Inc. , through issuance of Nonconformance Report W3-3304, identified that a section of tube steel fabricated from Heat Number 16600 had laminations. Eighty (80) feet of tube steel was utilized by Mercury in installations of seven (7) supports for the Steam Generator System. Also, Tompkins-bcckwith. Inc., utilized one hundred sixty (160) feet of this tube steel in the fabrication of twenty-six (26) hangers for the following systems:

Waste Management,- Chemical and Volume, Reactor Cooling, Safety Injection, Chilled Water, Component Cooling,'and Valve Supports. Approximately eighty-(80) feet of this steel has not yet been identified, and currently a search is being made to locate the remaining eighty (80) feet. This material was procured under Purchase Order Number WP3-5908 and supplied by Mills Alloy Co., Twainsburg, Ohio.

SAFETY IMPLICATIONS The extent of potential impact on safety-related systems is pending review of additional information as described below by Ebasco Site Support Engineering (ESSE).

CORRECTIVE ACTION-

. Subcontractors Mercury Company of Norwood, . Inc. , and Tompkins-Beckwith, Inc. ,

have been instructed to identify usage of all material as related~to original purchase order. A special transducer has been procured, and three (3) twenty-foot lengths of tube steel, Heat No.16600, have been ultrasonically tested to determine the characteristics of~the. laminar indications. ESSE is now in the process of determining what corrective action is to be initiated in order to resolve all deficiencies. It is estimated that an Interim Status Report based on these actions will be submitted to the USNRC by June 25, 1982.
