Appendix NOTICE OF VIOLATION Albion Community Hospital License No. 21-13987-01 i
As a result of the inspection conducted on April 22, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violations were identified:
1.
10 CFR 35.14(b)(5)(ii) requires that records of leak test results be kept in units of microcuries.
Contrary to the above requirement, as of the day of the inspection, your records of leak test results were in co'ints per minute rather than the required microcuries.
This is a Severity Level V violation (Supplement VI).
2.
License Condition No. 20 requires that licensed material be possessed and used in accordance with the statements, representations, and pro-cedures contained in certain referenced applications and letters.
I Item No. 7 of the referenced application dated March 30, 1981, states that the Medical Isotope Committee will meet at least quart 21y for the purpose of reviewing current usage of radioactive materials.
Contrary to the above requirement, you failed to hold quarterly meetings.

License Condition No. 16 states that all final source containers will be wipe tested for contamination.
Contrary to the above requirement, you failed to wipe test all final source containers since this requirement began on November 4, 1981.
This is a Severity Level V violation (Supplement VI).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avolo further noncompliance; and (3) the date when full com-pliance will be achieved. Consideration may be given to extending your response time for good cause shown.
