Adams County Hospital License No. 34-18796-01 As a result of the inspection conducted on April 22, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violations were identified:
1.
License Condition No. 15 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in certain referenced applications and. letters.
The referenced letter dated December 19, 1979, states that the Radio-isotope Committe shall meet not less than once in each calendar quarter.
Contrary to the above, the Committee did not meet' from the date the requirement began, January 3, 1980, to November 16, 1981, and not again until March 10, 1982. These periods exceed three months.
This is a Severity Level V violction (Supplement VI).

10 CFR 19.11(a) and (b), requires that current copies of Part 19, Part 20, license conditions, documents incorporated into the license, l'
license amendments and operating procedures be posted, or that a notice describing these documents and where they may be examined, be posted.
Contrary to this requirement, on the day of the inspection, neither the documents nor the notice were posted.
This is a Severity Level V violation (Supplement VI).

10 CFR 35.14(e)(1) requires that sealed calibration or reference sources possessed pursuant to 10 CFR 35.14(d) be tested-for leakage and/or contamination at intervals not to exceed six months.
Contrary to the above, a 211 microcurie cesium-137 sealed source leak tested by the manufacturer on February 26, 1980, and received by the l
hospital on May 9,-1980, was not leak tested until' July 17, 1981.
