Appendix NOTICE OF VIOLATION Grunau Company, Inc.
License No. 48-18383-01 As a result of the inspection conducted on April 30, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violations were identified:
1.
10 CFR 34.25(b) requires that you test each scaled source for leakage at intervals not to exceed six months.
Contrary to this requirement, records indicated that no leak test was performed on your iridium-192 source, No.87-041 from April 23, 1981 to December 29, 1981, a period greater than six months.
This is a Severity Level V violation (Supplement VI).
2.
10 CFR 20.407(b) requires that you submit to the Commission within the first quarter of each calendar year, a report of exposures recorded for individuals under your licensed program for the preceding calendar year.
Contrary to this requirement, as of the day of the ir.: pection, April 30, 1982, you had not submitted the required report for calendar year 1981.
This is a Severity Level V violation (Supplement IV).
3.
10 CFR 20.408(b) and 20.409(b) require that you make a report to the Commission, and to the individual involved, of the radiation exposure of each individual who has terminated employment.
Contrary to this requirement, as of the day of the inspection, April 30,
'1982, you have failed to provide the required report to the Commission, or to the individual involved, for an individual who terminated his employment in 1981.
This is a Severity Level V violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action 820 5-18 0 6/0 b

to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Consideration may be given to extending your response time for good cause shown.
