un This refers to your application dated April 9,1982, for renewal of the Certificate of Compliance No. 9086 for Model No. HM-100. Series 1.
Because the certificate consists of an application and several supplements, we request that you submit a consolidated application incorporating all previous submittals. The consolidated application need not include your QA program but should continue to include quality control requirements and operating procedures. The consolidated application (8 copies) should be sulmattted at least thirty (30) days prior to the expiration date of the certificate.
