e' Appendix NOTICE OF VIOLATION Burnham City Hospital License No. 12-03243-02 As a result of the inspection conducted on April 30, 1982, and in accordance with the NRC Enforcement Policy, 45 FR 9987 (March 9,1982), the following violation was identified:
License Condition No. 15 requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in application dated September 27, 1979.
Item No. 10 of the application states survey meters used in the radiation safety program will be calibrated at least annually.
Contrary to the above, the NRC inspector learned from statements of licensee representatives and review of calibration records that this requirement was not met.
Specifically, between March 22 and April 30, 1982, the licensee did not have a survey meter which was in current calibration.
This is a Severity Level V violation (Supplement VI).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
