APPENDIX A NOTICE OF VIOLATION Mississippi Power and Light Company Docket No. 50-416 Grand Gulf License No. CPPR-118 As a result of the inspection conducted on February 22-26, 1982, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified.

10 CFR 50, Appendix B, Criterion V and the implementing section of the Operational Quality Assurance Manual, MP&L-Topical-1, paragraph 5.4.1 require activities affecting quality be performed in accordance with docu-mented instructions, procedures or drawings. Grand Gulf Startup Manual, Section SUM 5000, paragraph 4.1.4.3 states that...When a step or section of the test cannot be completed, or when results obtained are not within the stated acceptance criteria, an Exception Sheet shall be completed to docu-ment the problem except as noted in 4.2.7.7 of SUM 7000 which states that...

There may be occasions when it is not possible to continue a test due to various plant conditions or due to technical or procedural problems. Should this occur, the Test Supervisor must carefully consider what actions to take, since invalidating preceding or subseqtent test data or subsequent test steps may result. Option C states that...If it is necessary to perform TIPE activities as described in 4.2.7.4 such as troubleshooting and/or corrections, the Test Supervisor shall execute a Retest Control Form.

Further, the third paragraph of step 4.2.7.4 states that...When an official test, or portion of a test, being performed is stopped (interrupted) due to problems, subsequent activities by the Test Supervisor to identify the problem are termed Test Interruption for Problem Evaluation (TIPE) acti-vities. Consisting primarily of troubleshooting and performing trial runs, these activities will be performed under an approved MWO, per requirements of Maintenance Section Procedure 07-5-01-5.

Contrary to the above, during the period of February 23 and 24,1982 (during the performancc of the official HPCS preoperation test) the test supervisor had two test interruptions as defined in the startup manual as stated above.

The first test interruption the test supervisor logged in the TIPE in his official log, but failed to fill out the retest control form.

The second test interruption the test supervisor failed to log in the TIPE and failed to fill out the retest control form as required by the startup manual as stated below.

This is a Severity Level VI Violation (Supplement II.F).

cause shown.
