n APPENDIX A NOTICE OF VIOLATION Veterans Administration Medical Center License'No. 10-09569-01 As a result of the inspection conducted on March 24, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9,'1982), the following violations were id9ntified.

License Condition 15 requires that licensed material be used in accord-

'ance with statements, representations, and procedures contained in -your appl.ication dated August 25, 1975, and letters dated November 12, 1975,

and January 22, 1976.

One procedure states that all survey instruments will be calibrated at least annually.

Contrary to the above, two survey meters in the Nuclear Medicine Department, specifically, the GM survey meter and the Cutie Pie ion chamber, were last calibrated October 1980 and were overdue for calibration as of October 1981.

This is a Severity Level V Violation (Supplement VI).

10 CFR 20.102(a) requires each licensee to obtain from any individual who will be furnished personnel monitoring equipment, prior to fi rst -- ent ry into the ' licensee's restricted area during employment, a written statement confirming either that the individual had no prior occupational dose during the current calendar quarter, or the nature and amount of any occupational dose received during the calendar quarter.

Contrary to the above, such information was not obtained in writing on.the new Nuclear Medicine Technician hired in September 1981.

This is a Severity Level V Violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit _to this office within thirty days of the date of this Notice, a written statement or-explanation in reply, including: (1) admission or ' denial of the alleged _ viola-tions; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance'will be achieved. Consideration may be given to extending your response time for good cause shown.
