LER 82-22/IT, Unit 1 Dear Mr. Haynes This letter is to confirm our verbal notification of your Mr.E. B. McCabe of our discovery that from 12-18-80 until 4-18-82 all Unit 1 operations in excess of 200 have been performed with a combined leakage rate of greater than 0.60 L f r all penetrations and valves subject to Type B and C tests a
(T.S. 3.6.1.2.b).
At 1400 on 5-3-82 it was confirmed that an error in Surveillance Test Procedure M-571-1, Local Leak Rate Tests, caused the total measured Type B and C leak rate to be compared to L versus 0.60 L as required by a
a T.S. 3.6.1.2.b.
A complete description of this event, including causes and corrective action will be addressed in a follow-up report.
