Appendix NOTICE OF VIOLATION Providence Hospital License No. 34-13995-01 As a result of the inspection conducted on April 14, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9,1982), the following violations were identified:
1.
License Condition 19 states that the licensee shall possess and use licensed material in accordance with the statements, representations and procedures listed in application dated March 25, 1981 and referenced letters. The application states that GM area surveys will be performed daily in generator elution, dose preparation and patient injection areas.
Contrary to the above, GM surveys of the areas are only performed on Mondays and Fridays.
This is a Severity Level V violation (Supplement VI).

License Condition 19 states that the licensee shall possess and use licensed material in accordance with statements, representations and procedures listed in application dated March 25, 1981 and referenced lette::s. The application states that instrument linearity tests will be performed on your dose calibration at least quarterly.
Contrary to the above, linearity tests are performed semi-annually.
This is a Severity Level V violation (Supplement VI).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall-be submitted under oath or affirmation.
Con-sideration may be given to extending your response time for good cause shown.
