Dear Mr. Keppler:
This letter constitutes the notification required by Reference 1 of a component supplied to the Lacrosse Boiling Water Reactor which contained a defect.
On April 26, 1982, ASCO Environmentally Qualified solenoid valve, catalog No.
206-380-2F, serial No. 48976J, was determined to have a defect. The coil retaining clip and fluxwasher (see attached sketch) were found loose inside the solenoid housing enclosure. With the retaining clip not installed, there is sufficient room for the coil to move vertically off the solenoid base sub-assembly approximately 1/4 inch, which could lead to coil overheating and eventually failure.
Examination of the retaining clip showed it was slightly misshapen. This solenoid valve was a spare and had not been installed in the plant.
All the ASCO EQ solenoid valves which had been installed prior to discovery of the defect were installed in locations where failure would have been in a safe mode.
All 39 ASCO Environmentally Qualified solenoid valves installed or to be installed in the plant were examined on April 27, 1982. Seventeen were of a different design and therefore not opened. Fourteen of the solenoid valves had retaining clips similar to the one discovered loose. All were properly shaped, but one clip was not fully in place. The solenoid containing this clip, 73-25-017, is one of two redundant solenoids which operate the inboard Containment Building Ventilation inlet damper. The ASCO solenoid was installed in the fall of 1980 and has operated without failure. The catalog number of this solenoid was NP831654. An electrician properly installed the clip, which was verified by the LACBWR Electrical Engineer.

The catalog numbers of these solenoids are 206-380-4U, 206-380-4RU, 206-380-5F and NP831664E.
Mr. Robert Brimer, LACBWR Electrical Engineer, informed Bob Klick, Simone Engineering, the solenoid vendor, and Dick Beverly of ASCO on April 26, 1982. The spare solenoid valve is being returned to ASCO for examination.
Initial notification to the Nuclear Regulatory Commission was made to Paul Byron, NRC Region III, on March 28, 1982, by L. Goodman, LACBWR Operations Engineer.
