This refers to the telephone conversation between Mr. Charles A. Hooker of this office and Mr. Glen R. Cook, Full Service Wireline, Ltd., on April 15, 1982, regarding the possession of a radioactive sealed source which is not authorized by your license.
Regarding the matter discussed, we understand that you have undertaken or will undertake the following actions:
(1) Discontinue the use of the 5.0 curie Am-241, Model NBHP, Serial Number NB646, sealed source.
(2) Apply for an amendment to your license to add the 5.0 curie Am-241, Model NBHP, Serial Number NB646, sealed source to your license.
(3) Ensure that the unauthorized source is secured and not used until your license'is amended by the NRC.
