NOTICE OF EXPIRED LICENSE 4
This refers to the telephone discussion between Mr. D. B. Spitzberg of this office and Mr. Rulon C. Cummings of your organization on April 5,1982.
NRC records show that the Byproduct Material License identified above has expired and no renewal application has been filed in accordance with Section 30.37 of Title 10, Code of Federal Regulations, Part 30.
From our discussion with your representative we learned that the byproduct material previously authorized by this license is now covered by a separate active license.
In order for us to terminate this license we need you to complete the enclosed Form NRC-314 and submit it within 30 days to the U. S. Nuclear Regulatory Commission, Office of Nuclear Material Safety and Safeguards, Washington, D.
C., 20555.
A copy of the completed forr should be sent to Region IV, U. S.
Nuclear Regulatory Commission, 611 Ryan Plaza Dr., Suite 1000, Arlington, Texas, 76011, Attention:
Glen D. Brown, Chief, Technical Program Branch.
