' tory Commission (NRC) Region I Office (King of Prussia, Pennsylvania anuary 18, 1982, at your Woodbridge, New Jersey Office.
The inspection concerned the i
loss of a radiographic exposure device containing 24 curies of iridium-192 on i
January 15, 1982 near Oil City, Pennsylvania.
The exposure device fell from a truck driven by a licensee employee and was later found and reported by a passerby. The inspection revealed the following violation:
an exposure device was transported without being secured to the transport vehicle, without being locked, and without the rear safety plug inserted.
Earlier inspections conducted by Region I personnel at a field site in Paulsboro, New Jersey on November 25, 1981, and at your office in Woodbridge, New Jersey on December 15, 1981 disclosed three other violations:
(1) failure to properly survey an exposure device, (2) failure to recharge a pocket dosimeter at the start of a work shift, and (3) failure to perform annual radiation response checks on pocket dosimeters.
The violations that were identified as a result of these inspections are listed in the Notice of Violation enclosed as an Appendix to this letter.
These findings were discussed on March 2, 1982 during an enforcement conference at the Region IV Office with Mr. Karl V. Seyfrit and other members of the Region.IV staff and with you and other members of the Consolidated X-Ray Service Corporation.
In order to emphasize the need for licensees to provide proper control of radio-active sources which have a high potential for serious exposure to workers and members of the general public, we propose the imposition of a civil penalty in the amount of Four Thousand Dollars for the January 15, 1982 violation related to this matter as set forth in Section I of the Appendix to this letter.

Consolidated X-Ray Service Corporation violations in the Appendix have been categorized at the severity level described in the NRC Enforcement Policy published in the Federal Register, 47 FR 9987 (March 9, 1982).
You are required to respond to the Appendix and, in preparing your response, you should follow the instructions specified in the Appendix.
Your written reply to this letter and Appendix will be the basis for determining whether additional enforcement actions are warranted.
The responses directed by this letter and the accompanying Notice are not sub-ject to the clearance procedures of the Office of Management and Budget as required i
by the Paperwork Reduction Act of 1980, PL 96-511.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter and the enclosure will be placed in the NRC's Public Document Room.
