Pacific Air Products Company Docket:
99900769/82-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on February 8-11, 1982, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to tne circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall inclurie' appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Nonconformances with these requirements are as follows:
A.
Quality Assurance Operating Procedure No. 10.0, paragraph 4.2, states in part, " Inspection will tag shipment with the following types of tags:
Green accepted (route to controlled storage).... "
Contrary to the above, shipments were not appropriately tagged by inspection, as evidenced by the observation of 12 ga. galvanized sheets (Heat No. 622365),
14 ga. galvanized sheets (Heat No. 61240) and 16 ga. galvanized sheets (Heat No. 619976) in the controlled storage area which had not been identified with the required green accept tags.
B.
Quality Assurance Operating Procedure No. 3.0, paragraph 3.1, states in part,
" Chief Engineer assures that design documents contain correct and adequate application of design criteria..."
Contrary to the above, the Chief Engineer did not assure that design d( uments (drawings) contained correct and adequate application of design criteria, as evidenced by the absence on Drawing Sheet No. 5445-14, Rev. 4 for Shop Order No.13830, Damper Nos.1-CBA-DP-27A and B, of required location of welds, joint configuration and weld sizes.

Quality Assurance Operating Procedure No. 10.0, paragraph 5.4 states in part, " Nonconforming items or products will be tagged and processed in i
accordance with Procedure No. 15.0."
Contrary to the above, an in process inspector did not tag an actuator mounting bracket on Damper No. 1-CBA-DP-27A as nonconforming, although observed by the NRC inspector to contain cracks at both ends of a bend.
