SCOPE:

This inspection was made as a result of the identification to Region IV of the Nuclear Regulatory Commission, that alterations had been detected in WPPSS-WNP 3/5 piping weld radiographs, in regard to mechanical enhancement of specified penetrameter 4-T hole images.

This inspection was accomplished by review of radiographs and pertinent correspondence and interviews with cognizant personnel.

Contrary to Criterion V of Appendix B to 10 CFR Part 50 and Section V, Article 2 of the ASME Code, radiography of pipe welds with wall thickness of less than 1/2 inch was not performed with a technique of sufficient sensitivity to display the penetrameter image and the specified hole for certain piping assemblies furnished to WPPSS-WNP 3/5.

During Ebasco review of radiographs for acceptability, it was identified that certain radiographs had been mechanically altered to include an artificial representation of the specified 4-T hole of the penetrameter.

Nuclear Regulatory Commission, Region V Referral of Penetrameter 4-T Hole Image Enhancement on Radiographs of Piping Welds Fabricated by Associated Piping and Engineering Corporation (AP&E)

It was discovered that some radiographs of Class 1 and 2 pipe welds, fabricated by AP&E for WPPSS-WNP 3/5, had been mechanically altered to include an artifical representation of the specified 4-T hole of the penetrameter.

The altered radiographs appear to be limited to pipe that is less than 1/2 inch wall thickness and where the radiographic technique used required the penetrameter selection be based on the single wall thickness of the pipe.

As the result of an investigation by the NRC Region IV office, it was established that radiographs had been altered, on occasion, for approximately eight years.

The method of alteration to the radiographs consisted of enhancing the image of the applicable penetrameter T-hole with a lead pencil, thereby making unacceptable or questionable quality radiographs appear to be acceptable when viewed under normal conditions.

AP&E in conjunction with Ebasco has reviewed all radiographs of pipe welds supplied to WPPSS-WNP 3/5 and those radiographs of welds that had been altered were reradiographed and interpreted with no rejectable discontinuities noted.

The investigation revealed that one person was responsible for altering the radiographs and he was discharged from the company immediately upon disclosure of this discrepancy.

AP&E issued a memorandum to all radiographic personnel stating that any person found responsible for the improper use of equipment or alterations of radiographs in any illegal way, will be subject to immediate termination with the company.
