- 12. OthCrarisON OF A bit NDMIN1/ MODIFICATION The purpose of this Modification No. 2 is to incorporate an additional requirement for provision of the Contractor's 1981 Low-Level Radioactive Waste Records and establish an option for provision of the Contractor's 1982 waste records.
Therefore, U.S. Ecology, Inc's letter of April 22, 1982 is hereby attached to and made a part of Contract No. NRC-02-81-047 which is modified as set forth below:
1.
Article I - Scope of Work is hereby modified as follows:
A.
Under Paragraph 1.0 Background, delete the second sentence in its entirety and substitute in lieu thereof, "The U.S. Ecology sites at Hanford, Washington and Beatty, Nevada are two of the three sites which received -
LLW for disposal during calendar years 1980 and 1981."
B.
Under Paragraph 2.0 Statement of Work, delete subparagraphs A. Work Required, B. Deliverables, and C. Progress Reports, in their entirety and substitute the following in lieu thereof:

Work Required The Contractor shall prepare two files, one containing copies of 1980 records and the second containing copies of 1981 records, of all radioactive material received and disposed of at the two sites.
For each site, the receipt and disposal records shall be organized into sections by month and the waste volume and curies of activity shall be tabulated for each section. The files shall include any other information accompanying the records that may provide further specific information about particular waste shipments (e.g., the date of receipt, the type, quantity and form of radioactive material received, the originator, the originating city, the shipping distance, and the type of package in which the radioactive material was shipped). Other information which would help NRC in understanding and interpreting the records shall also be included in the files, e.g., lists of specific isotopic activities, if such information is available. The Contractor shall furnish all the necessary qualified personnel, facilities, and services to perform the above-described effort.
B.
Deliverabl es The Contractor shall provide to the Project Officer no later than three (3) months following the award of this contract, the file for calendar year 1980, and no later than July 27,1982, the file for calendar year 1981.
Each file shall contain copies of all records and other information in accordance with A. Work Required.
These records shall be legible and delivered to NRC in chronological order with a covering letter.
C.
Progress Reports The Contractor shall provide verbal status via telephone to the Project Officer no less frequently than every two (2) weeks from the effective date of the contract to the date of delivery of the 1981 records. These reports shall consist of an assessment of progress made to date, as well as any problems related to l
the contract."

Delete Article II - Period of Performance in its entirety and substitute the following in lieu thereof:
" Article II - Period of Performance The period of perfomance for this contract shall commence on September 14, 1981 and all files shall have been delivered to the NRC and all work shall have been completed by April 30, 1983 "

Delete Article III - Consideration in its entirety and substitute the fo'llowing in lieu thereof:
" Article III - Consideration A.
The Contractor will be paid a total firm fixed price of $27,905.00 for full performance of the services required under this contract.
This firm fixed price is comprised of $13,600.00 for all services and deliverables associated with the 1980 records, and $14,305.00 for all services and deliverables associated with the 1981 records.
B.
In accordance with Clause No. 3 entitled " Payments" of the General Provisions for Fixed Price Technical Assistance Contracts, the Contractor may submit invoices for payment of the above amounts upon acceptance by the NRC of each calendar year's files containing the complete records as previously specified.
Payment by the NRC will be made after submission of said invoices in accordance with the attached billing instructions for fixed price contracts."

Incorporate the following additional Article XII entitled "Ohtion for Increased Scope of Work":
" Article XII - Option for Increased Scope of Work A.
At the option of the Government, the Scope of Work of this contract may be increased to include the provision of the Contractor's 1982 Low-Level Radioactive Waste Records to be compiled as specified under Paragraph 2.0 Statement of Work, subparagraph A. Work Required.
This option shall be exercised by the issuance of an order by the Contracting Officer citing the authority of this Article. This option may be exercised at any time within the contract's period of performance.
The Contracting Officer wil,1 give preliminary notice to the Contractor, not less than thirty (30) days prior to the last day of the term of this contract, of the Government's intention to axercise the option. Such preliminary notice shall not be construed as an exercise of the option, and will not bind the Government to exercise the option.
B.
The parties hereto agree that upon issuance of the order exercising
-this option, the following modifications will be made to' the contract schedule in effect as of the date such issuance is made:
(1) Article I - Scope of Work Paragraph 2.0 Statement of Work, Sub-paragraphs A, B, and C.will be modified to incorporate provision t
of the 1982 records.
(2) The period of performance specified in Article II - Period of Performance will be extended accordingly.
(3) The total fixed price amount of $27,905.00 will be increased as negotiated by the parties. "

Form. Claims shall be submitted on the payee's letterhead, invoice, or on the Government's Standard Fonn 1034 "Public Voucher for Purchases and Services Other Than' Personal," and Standard Form 1035 "Public Voucher for Purchases Other Than Personal -- Continuation Sheet." Printing Office, 710 North Capitol Street, Washington, DC 20801.
Number of Copies and Mailing Address. An original and six (6) copies shall be submitted to NRC offices identified below.
Frequency. The Contractor shall submit an invoice or voucher only after NRC's final acceptance for services rendered or products delivered in performance of the contract unless otherwise specifled in the contract.
Preparation and Itemization of the Voucher. The voucher shall be prepared in ink or typewriter (without strikeovers) and corrections or erasures must be initialed. It must include the following:
