Interim Report of 10CFR50.55(e) Item on 3/4" Carbon Steel Pipe Cracking (#64)

Dear Mr. Keppler:

On April 1,1982, Project Quality Assurance's Mr. E.L. Thompson, Acting Supervisor-Construction Quality Assurance, telephoned Mr. J. Konklin of NRC-Region III, to report on a problem with 3/4" Carbon Steel Pipe Crack-ing at the Fermi 2 site.

This item deals with a length of 3/4" Carbon Steel Pipe, that cracked during a flushing operation. The vendor was contacted even though the Certified Mill Test Report shows an acceptable hydrostatic test pressure of 2,500 psi.

The vendor responded that the flaw appeared to be caused by a defective extru-sion mandrul. Investigation is continuing to determine where similar material is located. Reportability will be determined by the safety implications of the installed pipe.

Another report on this item, either interim or final, is scheduled to be sent to you on or before August 27, 1982. If you have questions concerning this matter, please contact Mr. H.A. Walker, Supervisor-Construction Quality Assurance.
