NOTICE OF VIOLATION Tennessee Valley Authority License No. 16-19208-01 Golden Pend As a result of the inspection conducted on March 10, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violations were identified.
A.
License Condition 17 requires that licensed material be used in accordance with statements, representations, and procedures contained in your applica-tion dated November 9, 1979. One procedure requires that personnel dosim-etry devices be supplied to personnel using the moisture-density gauge.
Contrary to the above, as of the date of the inspection, no personnel dosimetry devices have been issued to personnel using the moisture-density gauge.
This is a Severity Level V Violation (Supplement IV).
B.
License Condition 15.A requires that each sealed source containing licensed material shall be tested for leakage and/or contamination at intervals not to exceed six months.
Contrary to the above, the two sealed sources cont'ained in the Model 3411B Troxler Gauge were leak tested on October 10, 1980 and subsequently on February 4,1982, an interval which exceeds six months.
This is a Severity Level V Violation (Supplement VI).
Pursuant to the provisions of 10 CFR 2.201, you are bereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged viola-tions; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
