April 2, 1982 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE -- PNO-IV-82-13 This preliminary notification constitutes EARLY notice of events of POSSIBLE 4

safety or public interest significance. The information is as initially is>

received without verification or evaluation, and is basically all that is known by NRC staff on this date.

SUBJECT:

HIGH PRESSURE INJECTION THERMAL SLEEVE DEGRADATION Arkansas Nuclear One. Unit 1 is in cold shutdown for a scheduled maintenance outage. Nondestructive (radiographic, ultrasonic,anddyepenetrant)examina-tion is being performed on the four High Pressure Injection nozzles.

Pre-liminary results are as follows:

Upstream buttons (thermal sleeve retainers) are missing. The upst q.

end of the thermal sleeve (TS) is worn. The TS has wall thickness //

reduction and is axially displaced upstream about 1 inch. There no indications on ultrasonic (UT) or dye penetrant (PT) examinati i_. C _; *, m

Radiography (RT) indicated a gap over about the upstreasa 50% of t gppfl 1982. * :

a tas rolled area. No indications on UT except one indication which is 6, CT.yQ[""

unchanged from 1978 data.

No indications of abnormal conditions on RT. PT results include thre e

exterior nozzle weld indications which must be evaluated further.

RT results indkated a possible 360 crack in the TS in the rolled area.

PT revealed no unacceptable indications. This is the nozzle used for normal makeup.

A Region IV inspector has been on site observing portions of the examinations and reviewing results. Other NRC offices are being kept informed of inspection results as they are obtained.

The licensee is obtaining repair recommendations from Babcock & Wilcox.

Necessary replacement parts are enroute to the site. Outage extension beyond i

the scheduled April 23, 1982, restart date is likely.

No media interest is evident. The licensee has not made a press release, f)

RIV does not plan a press release.
