Mr. J.
P. O'Reilly U. S. Nuclear Regulatory Commitsion Region II Suite 3100 101 Marietta Street, N.W.
Atlanta, GA 30303
Subject:
J.M. Farley Nuclear Plant Special Report-Unit 2
Reference:
Dear Mr. O'Reilly:
This report is being submitted as requi red by Techni-cal Specification 3.5.2.b.
At 2308 on 2-28-82 an inadvertent safety injection and reactor trip was experienced during an attempt to synchronize the unit.
The safety injection occurred when the main generator instantaneously picked up 300 MWe load which reduced the S/G pressure below the SI setpoint.
The maximum number of occurrences of actuation of the safety injection considered in the Westinghouse design transient analysis is fifty (50).
This is the second safety injec-tion experienced on Unit 2 at Farley while in a hot condi-tion (Modes 4 through 1).
Modifications have been made to the DEH program to prevent recurrence.
Also, at 2308 on 2-28-82, the boron concentration in the boron injection tank fell below the minimum required by Tech. Spec. 3.5.4.1 (7817 ppm versus 20,000 ppm requir-ed) due to the safety injection.
Batching of the boron injection tank was begun and the boron concentration was returned to the minimum Tech. Spec. requirements at 1955 on 3-1-82.
Yours very
