l APPENDIX A NOTICE OF VIOLATION Washington Public Power Supply System Docket No. 50-460 P. O. Box 968 Construction Permit Richland, Washington 99352 No. CPPR-134 As a result of the inspection conducted on January 18-22, 1982 and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),
the following violation was identified:
A.
10 CFR 50, Appendix B, Criterion V, and Section 17.1.5 of the PSAR state in part:
... activities af fecting quality shall be prescribed 4
i by documented instructions, procedures, or drawings...and shall be accomplished in accordance with these instructions procedures, or d rawi ngs.... "
Shurtleff and Andrews Procedure No. 2D, Revision 4 of November 8, a
1978 " Visual Welding Inspection" requires, in part, in paragraph 4.13, that the Ouality Assurance Inspector will inspect all field welds and that welds shall be acceptable if weld size is as shown on q
drawings and meet the requirements as shown in AUS D1.1-72, Section 3.6.
l Section 3.6 of AUS D1.1-72 provides quantitative criteria for fillet weld convexity.
Contrary to the above, a required 1/2 inch fillet weld,- haunch 4343A to embed, for beam 2-4271A on Package No. 207A-056, Sheet 6 was not the weld size shown on drawings, it was 3/8~1nch and had been accepted by the Quality Assurance Inspector who initialed the item (no date) in the work package.
Additionally, a 5/16 inch fillet weld on the beam seat for bean Bfi 4154A, shown as a 5/16 inch fillet weld for the lower horizontal 4 X 4 X 3/8 angle to embed plate on field sketch FS-207-1-4-79-001 cn Contract 9779.207 ffodification FBT Change 66, had convexity 1/16 inch in excess of the maximum convexity allowed and had been accepted on June 8,1981 by the quality assurance inspector by signoff on the Field Held Inspection Report, Form U-1, for Unit 1 Containment, j
print E147 for elevation 453'-10".
This is a Severity Level V Violation applicable to Unit 1.
Pursuant to the provisions of 10 CFR 2.201, Washington Public Power Supply System is hereby required to submit to this office within thirty (30) days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further itens of noncompliance; and (3) the date when full compliance will be achieved.
