NOTICE OF VIOLATIO!i U.S. Department of the Interior License No. 46-15903-01 Bureau of Indian Affairs P. O. Box 389 Wellpinit, Washington 99040 As a result of the inspection conducted on February 19, 1982, and in accordance with the Interin Enforcement Policy, 45 FR 66754 (October 7,1930),

the following violation was identified:

License Condition 13A requires that each sealed source containing licensed material, other than hydrogen-3, with a half life greater than 30 days and in any fona other than gas shall be tested for leakage and/or contamination at intervals not to exceed six months.

Contrary to the above requirement, the 10 millicuries cesium-137 and 50 millicuries americium-741 sources contained in a Troxler Model 2401, moisture density gauge, Serial No. 2584, were leak tested on June 23, 1979 and were not leak tested again until May 19, 1930, a period in excess of 11 months.

Also, the sources have not been leak tested since tiay 19, 1980 to the date of the inspection, a period in excess of 21 months.

The above item constitutes a Severity Level IV Violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, U.S. Department of the Interior, Bureau of Indian Affairs, Wellpinit Washington, is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
