,e Appendix NOTICE OF VIOLATION Kelsey-Hayes Company License No. 12-02360-02 As a result of the inspection conducted on March 16, 1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
10 CFR 20.105(b) requires that radiation levels in unrestricted areas be limited so that if an individual were continuously present in the area, he could not receive a dose in excess of 2 millirems in any hour or 100 millirems in any seven consecutive days.
Contrary to this requirement, radiation levels exceeded 2 millirems per hour in an unrestricted area.
Specifically, on the day of the inspection, radiation levels on the roof over the radiographic cell determined to be an unrestricted area, exceeded 2 millirems in a one hour period.
This is a Severity Level IV violation (Supplement IV).

10 CFR 20.105(b) requires that you make such surveys as may be necessary for you to comply with all sections of Part 20.
As defined in 10 CFR 20.201(a), " survey" means an evaluation of the radiation hazards incident to the production, use, release, disposal, or presence of radioactive materials or other sources of radiation under a specific set of conditions.
Contrary to this requirement, as of the date of license issuance, you failed to make such surveys as were necessary to assure compliance with 10 CFR 20.105(b), " Permissible levels of radiation in unrestricted areas."
Specifically, you failed to make adequate surveys of the radiation levels on the roof over the radiographic cell.
This is a Severity Level IV violation (Supplement IV).
3.
10 CFR 34.25(b) requires that each sealed source shall be tested for leakage at intervals not to exceed six months.
Contrary to this requirement, you failed to leak test sealed source Model No. 424-2 during 1981.
This is a Severity Level V violation (Supplement VII).

10 CFR 34.43(b) requires that a radiation survey be made after each radiographic exposure to determine that the sealed source has been returned to its shicided position. Such surveys shall include the entire circumference of the radiographic exposure device and the source guide tube.
Contrary to this requirement, your radiographers failed to perform surveys that were adequate to determine that the sealed source had returned to its shielded condition.
Specifically, surveys of the circumference of the exposure device were not performed in 1981 and through March 16, 1982.
This is a Severity Level V violation (Supplement VII).
5.
10 CFR 20.203(b) states that each radiation area shall be conspicuously posted with a sign or signs bearing the radiation caution symbol and the words Caution - Radiation Area.
Contrary to this requirement, the radiation area on the roof over the radiographic cell was not conspicuously posted. Specifically, on March 16, 1982, the signs bearing the radiction caution symbol had deteriorated to an unreadable state.
This is a Severity Level VI violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
