REPORT OF NON-COMPLlANCE WITH CONDITIONS OF NPDES PERMIT PERMIT NO. PA 0009920 REPORT NO. 82-02 Identification of Occurrence:

Between 0730 and 0800 on March 18, 1982, approximately 2500 gallons of water was accidentally overflowed from the secondary Waste Neutralizing Tank (WT-T-1).

As a result, water with a pH of 3-5 was discharged from the neutralizing tank into the plant discharge effluent stream. Based on conservative estimates of dilution both experimentally and by composke sampling at the main station effluent discharge point (DSN 001) the plant discharge pH limit of 6.0 to 9.0 was not violated.

Description of Discharge:

PS" 105: ic - intermittent discharge of waste water frcm the Unit 1 Waste Neutralizing Tank. The system is sampled prior to discharge out of the station's main discharge line (DSN 001). The Waste Neutralizing Tank is used as a collection and treatment facility.

_Cause of the Non-Compliance:

The cause of the overflow from DSN 105 can be attributed to operator error.

Duration of Non-Compliance:

An approximate duration time of 4-5 minutes, during which approximately 2500 gallens of water overflowed into DSN 001.

Corrective Action to Prevent Further Recurrence:

In shcrt term, the system was shut down and the overflow terminated. A visual inspection was done at DSN 001 to confirm no obvious environmental impact occurred. The operator involved was counselled on his inappropriate actions, emphasizing the gravity of this easily prevented violation. In the long term, this incident will be reviewed by all plant operations crews. The Neutralizer Tark High Level Alarm Response Precedure will be revised to add e::plicit instructions to ensure operator knowledge of overflow consequences. The IWT l

Operating Procedure and the Neutralizing Tank Dump Procedure will be modified i to include specific oper tor guidance to prevent recurrences.
