AEC't-80/2 64, 10/22/80 on Ma rch 27, 1980, Mississippi Power & Light Company notified Mr. F. Cantrell, of your of fice, of a Potentially Reportable Deficiency (PRD) at the Grand Gulf Nuclear Station (GGNS) construction site. The deficiency concerns Rosemount Model 510 DU Trip / Calibration Units and Model 1152 Pressure Transmitters.

Based on the results of our investigation we have determined that these deficiencies are reportable under 10CFR21. All details are provided in this sup-plement. Our Final Report under the provisions of 10CFR50.55(e) had previously been submitted as per the above referenced letter.

This deficiency was also reported under 10CFR21 by General Electric to the NRC in their letter dated March 21, 1980,

4. Nature of the deficiency .. . and the safety hazard which . . . could be created by such a deficiency ...:

A. Description of the Deficiency The Rosemount Model 510 DU Trip / Calibration Unit has common mode failure of an essential switch that determines the trip output logic. The mal-function reverses the logic, thereby providing an erroneous signal. The Model 1152 Pressure Transmitter has a common mode failure of a capacitor when the transmitter operating temperatere is greater than 175 F or when the damping potentiometer is rotated clockwise. The failure is manifested as a drop in transmitter output current up to 10% of the correct current.

The trip unit is used in conjunction with the pressure transmitter to provide high or low trip signals.

B. Analysis of Safety Implications Due to the inherent safety functions of these devices, failure would adversely af fect the safe operation of Grand Gulf Nuclear Station. The deficiency is applicable to both Unit I and Unit 2. As an example, when the Model 510 DU is used as a water level trip unit, the deficiency can result in indicating a high water level in the reactor when actually the water level is low. As an example, when the Model 1152 transmitter is used as a pressure transmitter, the deficiency can result in transmitting reactor pressure 10% lower than actual.

5. The date on which the information of such deficiency ... was obtained.

Mississippi Power and Light received information of the deficiency on March 26, 1980. We reported the deficiency to Mr. F. Cantrell of your office as a Potentially Reportable Deficiency (80/11) on March 27, 1980. Since that date MP&L has filed one (1) Status Report, one (1) Interim Report, and one (1) Final Report to inform the Commission of the progress and status of this deficiency. An evalustion for Part 21 has been completed and the MP&L

The Rosemount Model 510 DU Trip / Calibration Units and Model 1152 Pressure Transmitters are used at CGNS within the NSSS scope of supply.

We do not have knowledge of the location of defective equipment other than at GGNS.

7. The corrective action which has been taken . . . the name of the individual . . .

responsible for the action; and the length of time that has been ... taken to complete the action.

A. Corrective Actions Taken General Electric issued r'ield Disposition Instructions (FDI's) to repair defective Model 1152 Pressure Transmitters on site and issued FDI's to return Model 510 DU Trip Units to the manufacturer for repair.

Site Manager Mississippi Power and Light Company C. Length of Time to Complete Actions Mississippi Power & Light received information of the deficiency on March 26, 1980. For the Pressure Transmitters General Electric issued Field Disposition Instructions FDI-WAME for Unit I and FDI-WBIY f or Unit 2. For the Trip Units, GE issued FDI-WAMD for Unit 1 and FDI-WBIX for Unit 2.

The Unit 1 FDI-WAMD was completed on June 6,1981. Unit 1 FDI-WAME was completed on October 28, 1981. The Unit 2 FDI's are still open.
