4 APPENDIX A NOTICE OF VIOLATION Based on the results of the NRC inspection conducted on Dece=ber 7-9, 1976, it appears that certain of your activities were not conducted in full compliance with NRC regulations and the conditions of your. license as indicated below.
Item.A is an infraction and item B is a deficiency.

10 CFR 50 Appendix E, item H requires that the emergency plan contain provisions for training persons, other than e=ployees, whose assistance may be needed in the event of a radiation emer-gency.
10-CFR 19.12 requires that you train all individuals who work in any portion of a restricted area.
Specifically, the Indian Point Station Site Emergency Plan, paragraph 14.7, requires that members of the Verplanck and Buchanan Fire Departments be annually trained in the locations and boundaries of radiation areas and precautions required therein.
Contrary to the above requirements, such training was not provided to the Verplanck and Buchanan-Departments.
B.
Technical Specification 6.8.1 requires that written procedures be implemented.
Implementation procedure 1018, paragraphs 2.2.5 and 2.3.5 require that defective and missing items be replaced and so noted on the checkoff list, with the completed checkoff list-to be sant to the Radiation Safety. Director.
Technical Specification 6.10.1.b requires that records of inspections be maintained for 5 years.
Contrary to the above requirements, checkoff lists were not proper-ly completed for monthly-emergency equipment inventories conducted from January 1976 through Novembey 1976 in that missing items'were not noted as being replaced, defective items were not noted as repaired or replaced, and rec ~ords of several inventories were not available to-serve as evidence that inspections had been performed.
