Rodent damage has occurred in electrical penetration wiring and cables. The damage consists of chewed insulation on a number of wires, resulting in exposed and partially covered conductors. This condition is documented on CPCo NCR M01-9-1-165, SCRE-39 and CPCo letter Serial: CSC-5830.

RECOMMENDED ACTION * (Optional):

a)

Take immediate action to minimize any further rodent damage to penetration wiring (Construction) b)

Inspect, identify, and establish extent of all damaged wiring and cables.

c)

Initiate the appropriate actions to repair or replace the damaged parts of the electrical penetrations.

d)

Determine root cause and take appropriate corrective action to preclude recurrence.

e)EFERRED TO: interim report by February 16, 1982.

A) CPCo Letter, Serial CSC-5830, 12/3/81 B) CPCo NCR M01-9-1-165, 12/28/81 C) SCRE No. 39, 1/15/82 Introduction This report provides interim status and the course of corrective action required pursuant to MCAR 56.

Description of Deficiency During post construction surveillance inspection of the Class lE electrical penetration assemblies (EPAs), assembly 2Z135 revealed rodent damage to the outboard side module wiring. The damage consisted of chewed insulation resulting in exposed and/or severed conductors. Subsequent inspection of additional Class lE assemblies revealed damage of the same nature.

Probable Cause The deficiency is a result of rodents entering / nesting in the EPAs and chewing on module wiring. Access into the EPAs was via the cable feedthrough nipples.

Corrective Action To date the following corrective actions have been taken to resolve the subject deficiency.

A)

Construction has initiated a re-emphasis on rodent control including:

1.

Removal of all nesting and debris from the EPA sleeves 2.

Additional rodent baiting with revised location of the bait 3.

An increase in the frequency of surveillance by the rodent control subcontractor.

4.

Administrative control of the areas containing EPAs 4

Construction has comp'.eted its inspection of all EPAs used for Class 1E circuits to determine the extent of damage.

C) 1)

Engineering has obtained and accepted repair / replacement procedures from the EPA manufacturer. These procedures include Vendor Prints 7220-E-20-182-1 and 7220-E-20-183-1. The EPA manufacturer has also approved the use of field procedures FPE 7.000 " Cable Terminations" and FPE 8.000 "Non-Class lE Wire and Cable Splicing" for use in the repair of damaged EPA modules.

2)

Construction is repairing damaged module wires in accordance with the accepted procedures.

3)

Where extensive damage precludes repair, the damaged module will be replaced in accordance with the manufacturers procedures.

D)

The root cause has been determined to be a breakdown in site housekeeping and the unsealed cable entrances into the outboard EPA junction boxes. Appropriate corrective action to preclude reoccurrence will be addressed in the next interim report.

Safety Implications The damage consisted of chtwed insulation resulting in exposed and/or severed conductors. Exposed conductors could result in circuit shorts and severed conductors could result in circuit discontinuities, therefore, Class 1E equipment necessary to safely operate the plant may have failed to operate.

Forecast Date on Corrective Action a)

A list of all safety related circuits affected by rodent damage will be provided by March 1, 1982.
