Dear Mr. Wasilewski:

This refers to the special inspection conducted on June 25, 1993, of your facilities which were formerly occupied by Horizons, Inc. located at 2891-2905 East 79th Street, Cleveland, Ohio.

Licensed activities were previously authorized by Atomic Energy Commission (AEC) Source Material License No. C-3496 and Byproduct Material Licenses No. 27229 and 34-001947-01.

This inspection was conducted by Messrs. D. G. Wiedeman and G. M. McCann of i

this office. This also refers to the telephone conversation between Mr. D. G. Wiedeman and your legal counsel, Mr. John D. Karlovec, Esq., on l

August 3,1993, regarding the results of our confirmatory measurements and wipe tests for removable contamination collected during the inspection.

The results of our preliminary survey findings were discussed with you and Mr. C.

Estep at the conclusion of the inspection on June 25, 1993.

The enclosed copy of our inspection report identifies areas. examined during the inspection. The inspection consisted of a selective examination of f

records from the former license file, observations, independent measurements,

-l and interviews with selected employees of Lamotite.

Based upon this inspection, we have concluded that the former licensee's processing building, now currently used for equipment storage, was not decontaminated to a residual radiation level consistent with current NRC guidelines. The radiation levels found in these areas do not constitute an immediate health and safety problem for your employees and visitors of the building; however, we do expect remediation of the contaminated areas. Therefore, within 30 days from the date of this letter, please provide this office with your schedule. and plan to characterize the extent of the contamination and decontaminate the facility to current NRC criteria.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
