Response to Weaknesses Identified During 1993 Annual Emergency Exercise, NRC Inspection Report 93-05 Reference 2 transmitted three exercise weaknesses resulting from the 1993 Annual Emergency Exercise conducted on June 29, 1993 at the Fort Calhoun Station.
Attached is the Omaha Public Power District (OPPD) response to these weaknesses.

i WEAKNESS #1 Failure to make a complete initial notification of the General Emergency in a timely manner was identified as an exercise weakness (285/9305-01).

Response
Omaha Public Power District (OPPD) has completed a review to determine the cause of this weakness and as a result, OPPD has taken or will take the following corrective actions:
1.
OPPD has provided additional training to command and control positions in the Emergency Response Organization (ERO), re-emphasizing the importance of timely and complete notifications.
This training discussed the exercise weakness and re uirements of EPIP-OSC-2, " Command and Control Actions Notifications".
2.
A human factors review of EPIP-OSC-2, " Command and Control Actions / Notifications," and its associated notification forms will be performed.
The review will be completed and the procedure / forms will be revised, if necessary, by February 1, 1994.
WEAKNESS #2 The failure to notify promptly plant personnel of emergency conditions was identified as an exercise weakness (285/9305-02).
Response
Omaha Public Power District (OPPD) has completed a review to determine the cause of this weakness and_as a result, OPPD has taken or will take the following corrective actions:
1.
OPPD has provided additional training to command and control positions in the ERO, re-emphasizing the importance of notifying, and keeping informed, personnel onsite during an emergency.
This training discussed the exercise weakness and requirements of EPIP-OSC-2.

As discussed in the response to Weakness 285/9305-01, OPED will perform a human factors review of EPIP-OSC-2, " Command and Control Actions / Notifications" and initiate any changes deemed appropriate for enhancing the notification of both offsite and onsite personnel.
Specifically, the coordination requirements between the Technical Support.
Center (TSC) or the Emergency Operations Facility (EOF) and the Control Room to sound the appropriate alarms will be reviewed.
The review will be completed and the procedure revised, if necessary, by February 1, 1994.
WEAKNESS #3 Throughout the exercise, the inspectors observed examples of erroneous or incomplete information that was communicated to organizations and individuals in need of the information.
Poor information flow was identified as an exercise weakness (285/9305-03).
Response
OPPD has completed a review of the documentation prepared during and after the exercise by the participants and controllers.
As a result of the review, OPPD has taken, or will-take, the following corrective actions:
1.
For ERO positions which have the potential to man the Health Physics Network (HPN) under current procedures, OPPD is providing additional training on what type of HPN communications can be expected and the importance of verifying data being transmitted over the HPN.
This training will be completed by September 1, 1993.
In addition, the lesson plans for these ERO positions will be updated by November'1, 1993 to cover HPN communications.
2.
The ERO positions which currently transcribe Emergency i
Response Facility Computer System (ERFCS) data from the system to emergency status boards will be instructed to i
transcribe data as presented by the ERFCS, including any validity identifiers.
In conjunction with this action, an informational summary will be prepared and transmitted to ERO members on the meaning of, and subsequent actions l
necessary for, applicable ERFCS data validity identifiers.
These actions will be completed by October 1, 1993.

Good communication practices, along with examples of poor communication practices, will be presented.to ERO members
[
via a " lessons learned" document.
This document will address the items identified by the NRC Inspection Team, as well as other items identified in previous training drills.
An overall command and control communications network will be outlined to ensure that personnel keep appropriate ERO directors and coordinators informed of their actions and observations.
This document will be prepared and transmitted to ERO personnel by October 1, 1993.
