1993 FIELD EXERCISE ONSITE OBJECTIVES OBJECTIVE FACILITY

1. Demonstrate the ability of the emergency 1,2,3 organization to assess plant conditions and make appropriate emergency classification. ,

1993 FIELD EXERCISE ONSITE EXTENT OF PLAY A. The locations where various objectives will be demonstrated are indicated by numerical codes as shown in the table below:

Objective 2, Notifications will be in accordance with EPP-203 and the Communications Workbook. Roleplayers will be used for the ENS Line, HPN Line, and calls to the EPA.

C. Objective 4, NOSF Training Simulator will be used instead of the actual Control Room. Operational personnel will be ,

pre-positioned in the Simulator at the start of the exercise. Additional personnel who staff the Control Room will report to the Simulator.

D. Objective 4, personnel not selected to staff their facility should report to the LSC, sign in on the Manpower  !

Availability Roster, and then return to work.

E. Objective 6, Command and Control will be demonstrated by the ,

CR, TSC, and EOF only. All other facilities will only demonstrate direction and coordination capabilities.

F. Objectives 18 + 19, location of demonstration is dependent upon which facility initiates control of the Offsite Survey Teams.

G. Objective 18, use of protective clothing and respirators will be simulated by offsite teams. -

H. Objective 20, Control Room (Simulator) will not demonstrate this objective due to location of the simulator.

I. Objective 20, Security will run an Accountability Report; however, actual followup to reconcile missing personnel will not be performed.

J. Objective 21, evacuation and early dismissal of onsite personnel will not be demonstrated. If ordered, early dismissal and/or site evacuation will be performed as ,

follows:

  • an announcement will be made stating that an evacuation would normally be conducted, but no actual evacuation will be performed.
  • the evacuation alarm will be sounded.
  • Security and Radiation Protection should be stationed >

at appropriate monitoring locations.

K. Objective 26, the decision to implement KI protective measures will be demonstrated out of sequence in the EOF.

Issue of KI will then be demonstrated out of sequence in the OSC only.

L. Objective 27, all actions for the reentry and recovery of this accident will be discussed during a tabletop session in the EOF subsequent to the termination of the exercise.

M. Objective 28, if requested, Chemistry will perform a PASS, l

appropriate data will be provided by the scenario.

N. Objective 29, although shift change will not be demonstrated during this exercise, facilities will be requested to formulate a shift relief schedule.  ;

This scenario is comprised of a fire in Unit 2, a Loose Parts Monitor alarm followed by seizure of a Unit 1 RCP and 1% failed '

fuel, an earthquake >SSE causing damage to Unit 1 containment and several plant systems, and a LOCA which results in a radiation release directly to the environment. During the exercise, a non-contaminated and a contaminated injured man will also be simulated.

Initial Conditions Unit 1 is in the process of a reactor startup and is at 30%

power in its third fuel cycle, with 374 Effective Full Power Days utilization of the core. Unit 2 is operating at 100% 't power in its first fuel cycle with 35 Effective Full Power Days utilization of the core. All plant parameters are normal and stable.

The exercise is scheduled to begin at 07:00 a.m. on July 28, 1993.

Narrative Summary At ~0710, an injured person will be reported at the NOSF .

building requiring response by the onsite ambulance. A mechanical breakdown will disable the ambulance once it delivers the patient to Glen Rose Hospital. I At ~0730, a fire will break out in the Train A Centrifugal ,

Charging Pump on Unit 2. This fire will burn for approximately 15 - 20 minutes.

At ~0740, declaration of an ALERT due to the fire in Unit 2 safety-related equipment. ,

At ~0800, a Loose Parts Monitor alarm is received on Unit 1.

At ~0820, RCP #3 on Unit 1 will fault resulting in the loss of this pump.

Starting at ~0830, indications are observed of possible fuel failure due to the loss of #3 RCP.

At ~0840, a major earthquake occurs resulting in the

reactor trip,  ;

loss of all offsite power, i 10 - 15 gpm RCS leak, loss of SSW pump 1-02, failure of DG 1-02 to start,  ;

a breach of the Unit 1 Containment Building and a small )

radioactive release due to the RCS leak.

l At ~0930, the RCS leakage increases catastrophically resulting in a LOCA with an unmonitored release directly from the  !

Containment Building to the environment. This LOCA will result i in the declaration of a General Emergency.

At ~1130, a 45 degree wind shift occurs causing the release [

plume to shift affected zones.

At ~1230, one of the ERDC team members is injured and contaminated, requiring transport to Hood General Hospital, ,

At ~1330, containment pressure is reduced to the point that the release has stopped.

At 1500, the exercise will be terminated.

At 1530, there will be a tabletop session in the EOF to discuss reentry and recovery actions following this accident.

As it is located in your garage, severe damage will probably occur to your home if the. tanks become over pressurized and explode. All that you need to do is to open circuit breaker switch number 4 in the electrical panel in the pantry, and the compressor will be turned off.

FROM: Traveler - R.J. Even LOCATION: Access Control Points - EXTRA TIME: 4:00 PM MESSAGE: Your name is R.J. Even. Try to talk your way past the access control point. Don't say anything that might be interpreted as constituting a valid reason. Your purpose is to see if you can get in without having a good reason for entering.

Good luck. Don't give up the first time they say no.

ADDITIONAL NOTE TO SIMULATOR:

If unable to gain entry at the first access control point, go to the other access control' point and attempt to enter.

If denied entry at both points, proceed to the.cleburne Relocation Center and register as an evacuee.

If allowed entry at the first access control ~ point, the team will provide you with a self-reading dosimeter and fill out an Emergency Worker Radiation Exposure Record Form for you.

Make up whatever answers are needed. Drive past the access control point and come back in about'20 minutes from the other direction. After turning in your dosimetry, go to the other access control point and attempt to enter. When finished at the second access control point, go to the-Cleburne Relocation Center for monitoring and registration.

NOTE TO CONTROLLER: R.J. Even and' car are not contaminated.

TIME: 3:00 PM MESSAGE: You were walking across the parking lot at the relocation center looking for the entrance, not paying attention to the traffic. You suddenly noticed a car coming at you. At that instant, you stopped, putting almost all of your weight on your right leg. At the same instant, the left corner of the car's bumper hit your right leg at the same instant'the car stopped. You are knocked to the pavement, breaking your fall with your hands. You feel severe pain in your lower right leg and notice an abnormal protrusion under your pant leg while a little blood is seeping into the fabric.

You are conscious but stunned from the traumatic insult to your body. You feel a little dizzy, confused, and angry but never lose consciousness.

TO: AMBULANCE ATTENDANTS AT THE SCENE FROM: Medical Drill Controller LOCATION: Relocation Center - Cleburne TIME: 3:10 PM MESSAGE: Upon arrival you find the 45-year old stout male patient sitting in the parking lot of the relocation center, guarding his right leg. As you approach, the patient is reluctant to allow you to examine him and you notice false movement in his lower right leg. By patient survey you find an open fracture of the right tibia. The false movement of the right lower leg, deformity of-the lower leg below the fracture site,-and deformity at the left side of the knee suggests a fibula fracture and/or dislocation as well. Pulse is palpable at the ankle. Capillary refill in-the toes of the right foot occurs within two seconds. The patient can feel the pressure on his toes when capillary refill is checked. The patient is sweating profusely.

Abrasions exist on the palms of the patient's hands. No other injuries are obvious from the patient survey.

  • the right lower leg, the extremity deformity distal.to:the fracture site,.and the displacement of.the fibula;midshaft i and dislocationLof'the fibular head.. Pulse is palpable'  ;

distal to the fracture site.- Capillary refill in the-toes  :

of the right foot _ occurs within two seconds. The' patient-

  • can feel the pressure on his toes.when capillary refill is checked. Abrasions with. embedded debris exist on the palms of the patient's hands. Your evaluation reveals noLother injuries. The patient is diaphoretic. The patient is contaminated with radioactive materials (see_page 3).

Disrobal of the' patient significantly' reduces the'1evel of_  !

contamination on the patient. Decontamination washdown is ,

effective. Radiological contamination of the wound is'not l found. '
