During an NRC inspection conducted May 30 through July 10, 1993, a violation of NRC requirements was identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:

Unit 1 Technical Specification 6.8.1 requires, in part, that

" Written procedures shall be established, implemented, and maintained covering the... emergency power sources (e.g., diesel generator)."

Procedure 1104.036, Revision 32, Permaneret Change 1, " Emergency j

Diesel Generator Operation," Attachment A, "DG1 Valve Lineup,"

l requires that DG1 Air Dryer M-300A/B Bypass Valve F0-92A1 be normally closed.

Contrary to the above, on June 3,1993, Valve FO-92Al was found to be in i

the open position.

Unit 1 Technical Specification 6.8.1 requires, in part, that i

" Written procedures shall be established, implemented, and l

maintained covering... the applicable procedures recommended in l

Appendix A of Regulatory Guide 1.33, November 1972." Regulatory Guide 1.33, Appendix A, recommended that equipment control (e.g.,

locking and tagging) be covered by written procedures.

Procedure 1102.001, Revision 51, " Plant Preheatup and Precritical Checklist," Section 1 of Attachment E, " Category E Valve Position Verification," requires that Emergency Feedwater Pump P-78 Bearing Cooling Return Valve CS-1197 be locked open.

Contrary to the above, on June 8,1993, Valve CS-1197 was found to be throttled 2-1/2 turns open.

Full open for the valve is six turns open.

Unit 1 Technical Specification 6.8.1 requires, in part, that

maintained covering... surveillance and testing activities of

safety related equipment."

Step 2.9 of Procedure 1104.003, Revision 26, Permanent Change 1, f

" Chemical Addition," Supplement 2, " Boric Acid Pump P-39A Test,"

requires that Core Flood Makeup Tank T-110 Inlet Valves MU-51 and MU-52, if opened to perform the test, be closed upon test completion.

Contrary to the above, on June 24, 1993, Valves MU-51 and MU-52, which were opened to establish a flow path for testing Pump P-39A, were not

closed upon test completion.

These three examples constitute one Severity Level IV violation (Supplement I) (313/9306-01).

Pursuant to the provisions of 10 CFR 2.201, Entergy Operations, Inc., is hereby required to submit a written statement or explanation to the 3

U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV, 611 Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, and a copy to the NRC Resident Inspectors at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).

This reply should be clearly marked as a " Reply to a Notice of Violation" and i

should include for each violation:

(1) the reason for the violation,~or, if I

contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full 1

compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued to show cause why.the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
