SUBJECT: NRC INSPECTION REPUT 50-285/93-12 (NOTICE OF VIOLATION)
This refers to the inspection conducted by Messrs. R. Mullikin, R. Azua, and
D. Kelley of this office on June 24 through July 16, 1993.
The inspection
included a review of activities authorized for your Fort Calhoun Station
facility. At the conclusion of the inspection, the findings were discussed
with those members of your staff identified in the enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examination of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The purpose of the inspection was to determine
,
whether activities authorized by the license were conducted safely and in
accordance with NRC requirements.
Based on the results of this inspection, certain licensed activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). Two violations represent a failure to control switchyard
activities at your facility.
In addition, it was noted that inadequate and
ineffective communications between the work crews in the switchyard and the
licensed operator in the control room were contributing factors to this event.
We are particularly concerned regarding the unauthorized maintenance in the
switchyard since extensive information has been provided by the NRC to all
licensees regarding the need to ensure that switchyard activities are
controlled to prevent inadvertent offsite power interruptions. The violation
concerning an inadequate equipment tagout is important due to the potential
for personnel injury.
A management meeting has been scheduled for August 19, 1993, to gain a better
understanding of your view concerning the significance of this event, its
causes, and the actions you have taken and planned to ensure positive control
of switchyard activities.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing your response to this

inspections, the NRC will determine whether further NRC enforcement action is
