This refers to the inspection conducted by Messrs. T. Taylor, P. Prescott, and

E. Plettner of this office from May 18 through June 28, 1993. This inspection

i

included a review of authorized activities for your Quad Cities Nuclear Power

Station facility. At the conclusion of the inspection, the findings were

l

discussed with those members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of a selective examination of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

,

A special safety inspection was initiated during this report period to

,

'

investigate the root cause(s) for the Unit I high pressure coolant injection

'

(HPCI) rupture disc failure. The results of that inspection will be

documented in a separate inspection report. However, your inability to

i

'

implement long term corrective actions to eliminate the causes for safety

system failures at Quad Cities is a concern requiring prompt management

-

,

action. On April 21, 1993, we met at Quad Cities Station and discussed safety

system failures in general and specifically discussed the HPCI systems' past

performance.

In addition to the Unit 1 HPCI system rupture disc failure, Unit

2 HPCI was also found inoperable during the report period.

These events

-

identify a need for an in-depth self-assessment to improve the performance of

safety systems at Quad Cities.

You are requested to provide a written

,

response to this office within 30 days addressing your actions to identify the

causes for continuing safety system failures and the corrective actions to

q

prevent recurrence.

Further actions may be required following our evaluation

of the special safety inspection investigating the Unit 1 HPCI rupture disc

event.

During this inspection, certain other of your activities appeared to be in

violation of NRC requirements. A failure to properly maintain control room

records uas identified as described in paragraph 10 of the enclosed inspection

report. The violation was categorized as Severity Level V and is not being

cited because the criteria specified in Section VII.B of the " General

Statement of Policy and Procedures for NRC Enforcement Actions, (Enforcement

Policy, 10 CFR Part 2, Appendix C), were satisfied. This item resulted from a

lack'of operator attention to detail during shift turnovers.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter and the enclosed inspection report will be placed in the NRC

Public Document Room.
