NRC INSPECTION REPORT NO. 50-395/93-03 After further review of your response of March 25, 1993, to our Notice of l

Violation issued on February 25, 1993, we have concluded for the reasons described below, that the violation occurred as stated in the Notice of Violation.

The basis for denial of the violation was that the use of jumpers can be accomplished using " skill of the craft" without written procedures.

In this instance, jumpers were used to obtain a power supply from safety-related switch gear for testing relays.

Your response did not provide any additional information that justified the performance of these activities without written procedures.

The procedure for testing the relays had no reference for connecting jumpers to i

the switch gear.

Also the work order which was approved by the' control room supervisor for testing the relays did not reference the use of jumpers.

the operators were not aware of the overall scope of the work involved with relay testing their responsiveness to potential problems related to the testing could i

have been delayed.

A point of contention in your response was that this method of testing relays has i

been performed by SCE&G relay department personnel for over 27 years (13 years i

at V.C. Summer) without incident. While the testing included in your data may 1

have resulted in each relay as left being properly tested, we are not convinced that in the absence of a detailed procedure, your records necessarily would show all of the problems encountered before satisfactory results were obtained.

In your response you also dispute a statement in the Inspection Report concerning the use of jumpers for connecting test equipment to safety-related equipment.

Specifically you state that the jumpers were not ' connected to the test equipment but directly to the relay being tested.

The inspector considered the houstng device which enclosed the tested relay and to which the jumper cables were connected to be " test equipment". As indicated in the Inspection Report, since no details were provided for completing this portion of the task, there was no assurance of the consistent use of the jumpers.

l No additional information was provided to support your statement that the relay testing procedures met the requirements of the Operational Quality Assurance Plan. Paragraph 4.4.7 of the plan requires that written test procedures include the test method and any special test equipment required to conduct the test. As mentioned previously, the test procedure did not reference the method of obtaining a power supply by the use of jumpers.

In conclusion, we continue to believe the violation occurred as stated in the Notice of Violation. We are concerned that similar activities may be performed in the future using " skill of the craft" knowledge without procedural guidance.

Therefore, in accordance with 10 CFR 2.201(a), please submit to this office within 30 days of the date of this letter a written statement describing steps which have been taken to correct the violation and the results achieved, corrective steps which will be taken to avoid further violations, and the date when full compliance will be achieved.

We will examine the implementation of your actions to correct the violation during future inspections.
