NRC INSPECTION REPORT NOS. 50-352/93-14 AND 50-353/93-14
This letter refers to the inspection conducted by Messrs. N. Perry, and T. Easlick of this
office on May 25 through July 6,1993. The inspection included a review of activities
authorized for your Limerick Generating Station facility. At the conclusion of the inspection,
the findings were discussed with Mr. R. Boyce of your staff.

The inspection focused on issues important to ensuring the public health and safety. Areas
examined during the inspection are identified in the report. Within these areas, the

inspection consisted of observations of activities, reviews of documents and interviews with
plant personnel. The purpose of the inspection was to determine whether activities

authorized by the license wee conducted safely and in accordance with NRC requirements.

Based on the results of this inspection, certain of your activities appeared to be in violation
of NRC requirements, as specified in the enclosed Notice of Violation (Notice). The

violations are of concern because in one case it should have been prevented by pnor
_
corrective actions and in the other case corrective actions were not timely.

Although Sectic,n 2.201 provides for a written statement of explanation to this office within
20 days of your receipt of this Notice, we note that these violations have been corrected and
those actions, including your root cause analysis, were reviewed during this inspection.
Therefore, N response with respect to this matter is required.
Additionally, in three instances, we have exercised enforcement discretion because of your
identification of the events and your corrective actions and you met all the criteria of 10
CFR, Appendix C,Section VII B2. One involved two late NRC notifications required by 10

CFR 50.72. Anothu was due to a missed surveillance, for a primary containment isolation
valve, required by technical specifications. The last concerns the failure to implement a

technical specification action statement for an inoperable emergency diesel generator.

Although 2 cited and 3 non-cited violations were identified in this report, we noted that some
of the events occurred prior to this inspection period, but followup was not conducted or
completed until this inspection period. In all cases, our inspectors found the followup
,
investigations to be comprehensive. We encourage you to continue _to improve this process
and keep our inspectors informed of your progress.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and
its enclosures will be placed in the NRC Public Document Room.
The enclosed Notice is not subject to the clearance procedures of the Office of Management
and Budget as required by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
