SUBJECT: MILLSTONE COMBINED INSPECTION REPORT NOS. 50-245/93-16;
50-336/93-11; 50-423/93-13

This refcs to the safety inspection conducted by Mr. P. Swetland and others of this office on
May 19,1993 through June 29,1993, at Millstone Station in Waterford, Connecticut. The
,
preliminary findings were discussed with Mr. S. Scace, the Unit Directors, and others of
'
your staff at the conclusion of the inspection. Areas examined during the inspection are
described in the enclosed report. Within these areas, the inspection focused on issues
important to public health and safety, and consisted of performance observations of ongoing
activities, independent verification of safety system status and design configuration,
i
interviews with personnel, and review of quality records.
'
The inspectors identified that certain requirements of the Unit 2 license were not implemented
adequately during the inspection period. These included failure to declare certain chemical
and volume control system valves inoperable immediately after they failed an inservice test,
and failure to follow procedures regarding manipulation of radiation monitor valves. Three
violations of NRC requirements concernmg these problems are cited in the enclosed Notice of
Violation. We are particularly concerned about the repetitive nature of these licensee
identified violations, the lack of timely and comprehensive actions to prevent recurrence, and
the apparent ineffectiveness of your previous corrective actions. In your response to the
Notice of Violation enclosed with this letter, please provide your evaluation of the root causes
leading to the violations and those remedial measures you determine to be necessary to
'
preclude future procedure adherence problems.

You are required to respond to the enclosed Notice of Violation and should follow the
instructions in the enclosed Notice when preparing your response. In accordance with
10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and the enclosed
Notice will be placed in the NRC Public Document Room. The responses directed therein
are not subject to the clearance procedures of the Of6ce of Management and Budget as
required by the Paperwork Reduction Act of 1980, Public Law 96.511.
During this inspection we also evaluated your self-identification and correction of problems
related to a technical specification high radiation area boundary violation and the lack of
.
documentation of certain radiological surveys. We determined that your corrective actions
'
for these issues were acceptable and the specific problems had minimal safety consequences.
Therefore, in accordance with Section VII.B of the Enforcement Policy, enforcement
,
discretion was exercised and violations were not cited for these cases.
We also identified at Unit '2 weaknesses in your self-assessment of events concerning
-
exceeding the design basis temperature limits of two safety-related cooling water systems
i
during main condenser thermal backwashing; watchstander performance regarding an
inoperable liquid effluent radiation monitor; and use of incompatible test equipment during
surveillance testing of engineered safety features undervoltage bistable modules. At Unit 3
!
we identined that weak initial review of industry operating experience resulted in inadequate
,
testing of the supplementary leak collection and release system; and that your evaluation of
emergency diesel generator fuel oil storage capacity did not recognize that the failure to
satisfy the plant design basis was an unreviewed safety question. I believe these iss";.,
