Subject:
PROPOSED IMPOSITION OF CIVIL PENALTY - $5,000 This is to inform the Commission' that a Notice of Violation and Proposed Imposition'of Civil Penalty in the amount of $5,000 will be issued on or about July 28,-1993 to Tuls-Camma Ray, Inc., Tulsa, Oklahoma. This action is based on a Severity. Level.III problem involving the loss of'a radiography camera.

.from a licensee vehicle.
The camera'was recovered by a member of the public and returned to the iicensee within an hour of the incident. No adjustments-to. the base civil'r,enalty of $5,000 were considered appropriate.
It should be noted that the licensee has not been specifically informed of the enforcement action. The schedule of issuance ~and notification is:

The state of Oklahoma will be notified.
i The licensee has 30 days from the date of the Notice in which to respond.
