JUL 211993 Docket No. 50-373 b

Docket No. 50-374 Commonwealth Edison Company ATTN:

Mr. L.0. DelGeorge, Vice President Nuclear. Oversight and Regulatory Services Executive Towers We'st III

'1400 Opus Place, Suite 300 Downers Grove, IL 60515

Dear Mr. DelGeorge:

Enclosed for your review, before our scheduled meeting of August 20, 1993, is the Initial SALP 11 Report for the LaSalle Station Nuclear Plant, covering the period January 1, 1992, through May 31, 1993.

In accordance with NRC policy, I have reviewed the SALP Board Assessment and concur with their ratings.

It is my view that your conduct of nuclear activities in connection with the LaSalle facility was satisfactory.

During this assessment period, overall performance at LaSalle Station declined. The decline was attributed to a general lack of management aggressiveness in identifying and resolving problems which resulted in decreased ratings in the areas of Plant Operations and Emergency-Preparedness.

This was also evident in the area-of Safety Assessment / Quality Verification which remained a Category 2, after considerable discussion by. the SALP board.

You are urged to carefully evaluate the root causes for performance deficiencies and reverse performance declines. Specific highlights of the report are as follows:

1.

The Plant Operations area declined to a Category 2 rating. Although operator response to transients continued to be excellent, the lack of aggressiveness in problem identification and the adverse personnel error trend led to the decline.

2.

The Radiological Controls area retained a Category 2 rating. Good programs for as low as reasonably achievable and water quality were noteworthy.

However, source term reduction initiatives were implemented slowly and impacted station dose.

3.

The Maintenance / Surveillance area retained a Category 2 rating.

Equipment failures and maintenance deficiencies offset some notable successes in problem areas.

4.

The Emergency Preparedness area declined to a Category 2.

Resource reductions contributed to out-of-date procedures and slow progress on other issues.

JUL 211993 6.

The Engineering / Technical Support area retained a Category 2 rating.

Positive initiatives were taken in several areas.

However, inadequate root cause analyses led to plant transients.

7.

The Safety Assessment / Quality Verification area retained a Category 2 rating but performance declined.

The decline appeared to be due to management's lack of aggressiveness and timeliness in dealing with known problems or previously identified weaknesses.

At the SALP meeting, you should be prepared to discuss our assessments and your plans to improve performance in the area of Safety Assessment / Quality Verification.

The meeting is intended to be a candid dialogue wherein any comments you may have regarding our report are discussed. Additionally, you may provide written comments within 30 days after the meeting.

Your comments, a summary of our meeting, and my disposition of your ~ comments will be issued as the Final SALP Report.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter and the Initial SALP Report will be placed in the NRC's Public Document Room.

Should you have any questions concerning the Initial SALP Report, we would be-pleased to discuss them with you.

Sincerely, Oribnc! C1:neny hhn D.th:Un John B. Martin Regional Administrator
