NRC REGION I INSPECTION NO. 50-333/93-12

This refers to the results of the routine resident safety inspection conducted by Messrs. W.

Cook and J. Tappert from May 23,1993 to June 26,1993 at James A. FitzPatrick Nuclear

Power Plant, Scriba, New York. A summary of the inspection findings was presented to you -

and members of your staff at the conclusion of the inspection.

This inspection was directed toward areas important to public health and safety. Areas

examined during the inspection are described in the NRC Region I inspection report, which -

is enclosed with this letter. _ Within these areas, the inspection consisted of observation of

activities, interviews with personnel, and document reviews.

Observed performance by your plant staff during this inspection period was good. One of

the problems your staff identified during the post-trip review of the April 20 reactor scram

that involved exceeding Technical Specification (TS) 3.6.A.3 (cooldown and heatup rates)

and TS 3.6.A.4, Figure 3.6-1 (reactor vessel pressure-temperature curves) were reviewed in

detail. We note that control room operator response to the scram and actions to stabilize

reactor vessel level were good. However, due to insufficient training and sensitivity to the

potential for reactor vessel bottom head temperature stratification, the rapid cooldown and

subsequent heatup of the bottom head area was not minimized. However, your corrective

actions for this event were prompt and comprehensive and the safety consequences of the

thermal transients were minor. Therefore, the Technical Specification violations are not

being cited in accordance with the provisions of 10 CFR 2, Appendix C, Section VII.B.2 of

the Enforcement Policy.
