Dear Mr. IIaughney:
I am writing in regards to the Quality Assurance Requirements for the transportation on radioactive materials. Please find enclosed the Great Lakes Testing, Inc.'s Transportation Quality Assurance Program and Company Structure. In pursuit of the phone conversation I had with Tom Matula, Great Lakes Testing, Inc. has received a Material Licenses and we are waiting our Quality Assurance approval to begin work. An expeditiousness processing of this application would be greatly appreciated.
In addition to the above mentioned I am submitting an Intent To Use list. This hst contains all of the packaging units Great Lakes Testing, Inc. shall use when transporting radioactive material.
This Cover Letter, Quality Assurance Program, and Company Structure will be sent by FAX to Tom Matula and Charles J. IIaughney on July 9,1993. A hard copy of these documents along with our $250.00 application fee will be sent out via Federal Express on July 9,1993. If you have any questions please feel free to call me at (414) 336-3554.
