NRC INSPECTION REPORT 50-313/93-05; 50-368/93-05
This refers to the inspection conducted by Ms. L. J. Smith, and the inspectors
l
identified in the enclosed report, of this office on April 18 through
!
May 29, 1993. The inspection included a review of activities authorized for
your Arkansas Nuclear One, Units 1 and 2, facility. At the conclusion of the
inspection, the findings were discussed with those members of your staff
'
identified in the enclosed report.
Areas examined during the inspection are identified in the report.
Within
!
these areas, the inspection consisted of selective examination of procedures
!
and representative records, interviews with personnel, and observations of the
activities in progress.
Your staff identified that a " Locked High Radiation Area" was unguarded and
unposted for a short period following a heat stress event. This was in
violation of Units 1 and 2 Technical Specification 6.8.1.a.
It was also
identified by your staff that respiratory protection equipment was used after

the period for which the inspection and operational check of the equipment
would have been valid and this was in violation of Units 1 and 2 Technical Specification 6.8.1.a.
These violations are not being cited because the
criteria in paragraph VII.B.2 of Appendix C to 10 CFR Part 2 of the NRC's

" Rules of Practice," were satisfied. These appear to be isolated occurrences
and your staff took prompt and effective immediate corrective actions.

Comprehensive actions to correct the problem were planned.
It is our
understanding that, during a postexit meeting conducted on Jum 10, Mr. Mike

Sellman, General Manager, Plant Operations, made the commitme cs listed in
Sections 6.3 and 6.4 of the enclosed report to prevent recurrence of these
problems.
