Dear Mr. Broughton and Dr. Long:

SUBJECT: NRC INSPECTION REPORT NOS. 50-289/93-13 AND 50-320/93-06

This letter transmits the findings of the resident safety inspection conducted by

Ms. M. Evans and Mr. D. Beaulieu from May 11,1993 - June 21,1993, at the Three Mile

Island Nuclear Station. The inspectors based these findings on observation of activities,

interviews, and document reviews and discussed them with Mr. T. G. Broughton and other

members of your staff at the conclusion of the inspection period. Inspector observations

indicated safe and conservative operation.

During the inspection period, a valve mispositioning event involving a building spray system

flow instrument was discovered. The mispositioning did not affect the operability of the

building spray system. However, we are concerned that there have been several recent

instances where Instrumentation and Controls technicians have not properly returned

equipment to service following maintenance or surveillance activities. We understand that

you are performing a formal root cause analysis to determine if the incidents have a common

root cause so that appropriate corrective actions can be taken. If this understanding is not

correct, please contact me promptly.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and

its enclosures will be placed in the NRC Public Document Room. We appreciate your

cooperation.
