Dear Sir:
This notification is being sent in accordance with
- 10CFR26, Appendix A, Section 2.8 (e) (4).
On May 28,-1993, a blind urine sample positive for Amphetamine was reported negative by the laboratory (Roche Biomedical Laboratories, Raritan, New Jersey).
Roche Biomedical Laboratory was advised and promptly investigated the matter.
The results of that investigation are summarized in i
the reference letter attached.

'I Thank you for your phone call on Tuesday, June 1, alerting me to a.
potential false negative result for specimen number 148.-718-0178.
i Based upon our discussion and at your written. request, I had the sample re-analyzed as' part of my investigation.
The results-obtained upon repeat screening indicated.the presumptive presence i
of an amphetamine compound.
An amended report will be generated _
i upon completion of testing.
j My investigation lead me to conclude that the technologist

performing the secondary FPIA rescreening handled the-incorrect aliquot.
l Corrective action will consist of a second verification of aliquots required for secondary rescreening against the internal aliquot i
chain of custody worklist.
All staff have been met with and appropriately counselled.
