L Southern Nudear Operating Company the s othemtectnc sprem June 28, 1993 Docket Nos.
50-348 50-364 U.S. Nuclear Regulatory Commission Attn.: Document Control Desk Washington, DC 20555 Joseph M. Farley Nuclear Plant Report of Unsatisfactory Performance Testina Gentlemen:
In accordance with 10 CFR 26 Appendix A, paragraph 2.8, Southern Nuclear Operating Company (Southern Nuclear) requires that blind performance test specimens be submitted to the HHS-certified laboratory used to-confirm the presumptive positive on-site screens. On June 2, 1993 a known blind positive sample was forwarded from the Southern Nuclear screening facility at the Joseph M. Farley Nuclear Plant to SmithKline Beecham Clinical Laboratories (SKBL) located in Atlanta, Georgia for confirmation testing.
On June 4,1993 the sample was reported as negative by the Medical Review Officer. An investigation of the discrepancy was begun immediately upon receipt of the Medical Review Officer report.
The subsequent investigation determined a suppression in discrimination above the cut-off for the THC-100 assay used by SKBL which was isolated to channel one on the SKBL Olympus screening instrument. Corrective actions by SKBL include switching channels on the Olympus machine and requesting that the manufacturer repair the malfunctioning channel.
Enclosed are the findings and supporting documents by SKBL and ElSohly Laboratories.
This letter is considered to satisfy the reporting requirements of 10 CFR Part 26 appendix A, paragraph 2.8.
Respectfully submitted, 020045 d,mL4 J. D dard (Exe ve Vice President JDW/JMG Enclosures o
