NRC REGION I INSPECTION NO. 50-333/93-10

This refers to the results of the routine resident safety inspection conducted by Messrs.

W. Cook and J. Tappert from April 18,1993 to May 22,1993 at the James A. FitzPatrick

Nuclear Power Plant, Scriba, New York. A summary of the inspection findings was

presented to you and members of your staff at the conclusion of the inspection.

This inspection was directed toward areas important to public health and safety. Areas

examined during the inspection are described in the NRC Region I inspection report, which

is enclosed with this letter. Within these areas, the inspection consisted of observation of

activities, interviews with personnel, and document reviews.

Overall performance during this inspection period was good. We view your staff's response

to the intake structure ice blockage incidents to be thorough. During the post-trip review for

the April 20 reactor scram and as detailed in LER 93-09 issued at the end of this inspection

period, your staff identified inadequate logic testing of the high pressure coolant injection

pump discharge isolation valve and concerns with cool down and heat up rates in the reactor

vessel bottom head area during scram recovery. Our inspectors will examine these issues in

a subsequent inspection period.

Your May 14 self-identification of an inoperable smoke detector system in the East and West

cable tunnel led to a violation of Technical Specification 3.12.E.2. We note that your

corrective actions for this issue were prompt and comprehensive, the event was not of a

-

recurring nature, and that the actual safety significance of the event was minor. Therefore,

the Technical Specification violation is not being cited in accorc'ance with the provisions of

10 CFR 2, Appendix C, Section VII.B.2 of the Enforcement Policy.
