SUBJECT: NRC INSPECTION REPORT 50-498/93-17; 50-499/93-17

This refers to the inspection conducted by Dr. D. Blair Spitzberg and other

members of an NRC team during the period June 7-10, 1993. ~ The inspection

t

included a review of activities authorized for- your South. Texas' Project

,

Electric Generating Station facility. At the conclusion of the inspection,

the findings were discussed with you and those members o'r your staff -

.

identified in the enclosed report.

Areas examined included the implementation of your emergency plan and

procedures during your annual emergency exercise. Within these areas, the

inspection consisted of selective examination of procedures and representative

records, interviews with personnel, and observations by .the inspectors. The

l-

inspection findings are documented in the enclosed inspection report.

Six exercise weaknesses were identified during the inspection. Three

,

weaknesses discussed in Section 3.1 of the enclosed inspection report were, in

'

general, based on activities observed in the' Technical Support Center. The-

weaknesses related to the failure to recognize plant conditions' corresponding

to a General Emergency; poor performance in providing technical assessment,

diagnosis, and mitigative activities; and for insufficient administrative

,

staffing in the Technical Support Center and the failure to obtain additional

l

staffing or to reassign the missing staffs' responsibilities. ' A weakness is

l

discussed in Section 4.1 related to unnecessary delays in providing proper

treatment and removal by ambulance of a simulated injury victim. An exercise

weakness discussed in Section 6.1 was identified for several problems

associated with the issuance of complete and accurate notification messages.

This was a repeated weakness from the previous exercise conducted in 1992.

Finally, in Section 8.1 a weakness is discussed for failure of your

,

self-critique process to identify or properly characterize.several areas in-

need of corrective action.

-Exercise weaknesses according to 10 CFR Part 50, Appendix E.IV.F.5, are

inspection findings that. need to be corrected by the licensee. You are

requested to provide ~ this office with a description of your corrective
