SUBJECT:

NRC INSPECTION REPORT 50-188/93-01

This refers to the inspection conducted by Dr. J. B. Nicholas,

,

Mr. T. W. Dexter, and Dr. D. B. Spitzberg of this office during the period

May 24-28, 1993.

The inspection included a review of activities authorized

for the Kansas State University Nuclear Reactor Facility.

At the conclusion

of the inspection, the findings were discussed with you and those memoers of

the university staff identified in the enclosed report.

Areas examined curing the inspection are identified in the report.

These

areas included the reactor operations, nuclear material control and

accountability, radiation protection, emergency preparedness, and physical

security programs. Within these areas, the inspection consisted of selective

examination of procedures and representative records, interviews with

personnel, and observation of _ activities in progress. The results of this

inspection are documented in the enclosed report.

Based on the results of-this inspection, certain licensed activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

The violation is of concern because the security system

to the nuclear reactor facility appeared not to perform as required by the

Physical Security Plan.

During this inspection, it was identified that a licensed reactor operator was

not at the reactor console at all times the reactor was not secured, and this

was a licensee-identified violation of Technical Specification H.4.

It was

identified that the reactor pool water-was not tested for conductivity at

least weekly,-and this was a licensee-identified violation of Technical

Specification C.2.

It was identified that the nuclear reactor facility

security system was not alarmed when the security area was unattended, and

this was a licensee-identified violation of the Physical Security Plan.

These

violations are not being cited because the criteria in paragraph VII.B.2 of

Appendix C to 10 CFR Part 2 of the NRC's " Rules of Practice," were satisfied.

These violations were isolated occurrences and the licensee's staff took

prompt and effective corrective actions to correct the problems.

'

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and its enclosures will be placed in the NRC Public Document Room.

However, in accordance with 10 CFR Part 2.790(d), the documentation enclosed

concerning your Physical Security Plan (Attachment 2 to the Appendix B) is

exempt from disclosure.

Therefore, this Attachment will not be placed in the

Public Document Room.

Should you have any questions concerning this inspection, we will be pleased

to discuss them with you.
