SUBJECT: NRC INSPECTION REPORT 50-445/93-19; 50-446/93-19
This refers to the inspection conducted by Mr. D. N. Graves, and the
inspectors identified in the enclosed report, of this office, on April 18
through May 29, 1993.
The inspection included a review of activities
authorized for the Comanche Peak Steam Electric Station, Units 1 ar.d 2,
facility. At the conclusion of the inspection, the findings were discussed
with you and those members of your staff identified in the enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The results of this inspection are documented on
pages 1-2 in the enclosed report.
One unresolved item was identified with respect to a Unit 2 power ramp rate
increase that exceeded the limits established in the operating procedure.
This matter is discussed in Section 2.2 of the enclosed inspection report. We
concur with your staff's assessment that the power increase only exceeded fuel
warranty limits and did not approach Technical Specification limits.
It was
also found that management's expectations for supervising, controlling, and
documenting the power increase were clearly established; however, we are
concerned with the personnel performance aspects of this event.
In
particular, the licensed operators' performance appeared to be similar to that
which contributed to the loss of spent fuel pool cooling event in May 1992.
In order for the NRC to further understand this issue, a management meeting
has been scheduled for July 8,1993, at the site.
We request that the meeting
include an overview of the event, a review of the licensed operators'
performance including supervisory oversight, an evaluation of the similarities
of this event with the previous loss of spent fuel pool cooling event, and a
discussion of the effectiveness of previous corrective actions and any
additional corrective actions planned and/or implemented.

that you discuss any performance based reviews that have been performed in the
