SUBJECT: COOPER NUCLEAR STATION (CNS) SHUTDOWN RISK AND OUTAGE

MANAGEMENT INSPECTION (NRC INSPECTION REPORT 50-298/93-201)

I am forwarding the report of the shutdown risk and outage management

inspection conducted by the U.S. Nuclear Regulatory Commission-(NRC) staff

}

from March I through March 5, and April 8 through April 16, 1993. The-

,

activities inspected were authorized by NRC Operating License DPR-46 for the

!

Cooper Nuclear Station. At the conclusion of the inspection, an exit meeting

was held where the team discussed its inspection findings with members of your

i

staff.

j

.

During the first phase of the inspection, conducted before the 1993 refueling,

~

outage, the team examined the adequacy of your outage planning with respect to

minimizing the risk of accident sequences during shutdown conditions. During

,

-

the second phase, conducted during the outage, the team examined the

implementation of your outage plan.

i

The team found that you had developed and were implementing an outage plan -

that considered the risk associated with plant shutdown evolutions. Specific

'

strengths included a reduction of overall personnel exposure during the

.

!

shutdown evolution, good operator awareness of equipment available to mitigate-

ever.ts which impact shutdown safety, and strong control over fuses removed by

'

clearances. The team also viewed your shutdown risk self-assessment review as

j

thorough, but noted that not all corrective action recommendations had been

completed.

!

i

The team identified deficiencies in your programs in the 'following' areas: (1)

'!

procedural nonadherence associated with maintenance, operations, and

administrative activities; and (2) an apparent Technical. Specification non-

,

compliance involving movement of the reactor vessel head and upper internals

1

over the core without an operabla secondary containment.

j

Areas for potential improvement and program enhanceme'nt' include the following::

'{

(1) continued management attention to the implementation of your self-

t

assessment recommendations; . (2) more specific policy and implementation

a

-guidance for the shutdown safety program; and (3) better work planning and-

l

implementation strategies to reduce the number of events which continue to

f

impact shutdown safety functions.
